Breast Implant Removal

CLOSE X
This is a 28 year-old patient who wanted her implants removed. She no longer cared for them and felt that the implants were contributing to more back pain for her. The patient also wanted to start trying to get pregnant so she wanted to get the surgery out of the way. She was initially interested in a breast lift and fat transfer at the time of the removal but after my discussion with her, she decided to see how she would bounce back with the implant removal alone. After all, she had very youthful skin and tissue and had never undergone pregnancy and breastfeeding so there was a good chance that her breasts would restore their shape without additional procedures. She had 375cc breast implants removed in the operating room and her breasts bounced back to looking natural and beautiful. She was so happy that she didn’t have to undergo additional lift and fat transfer to achieve the outcome that she did.

Procedure

This is an active 30 year-old patient who had silicone breast implants placed ten years prior to her presentation. She started to feel her implants getting tighter, which was consistent with capsular contracture. Although she was hesitant about the size she would be post-removal, she knew for sure that she did not want them anymore. She underwent removal of 550cc silicone implants and thick capsules and concurrently underwent a “donut” mastopexy to help restore symmetry to her nipple areolas. She was baseline very fit which meant minimal donor sites to harvest fat from. However, in thin patients, even small amounts of fat can be helpful. I was able to graft 60cc of fat into each breast to help shape the breasts to make up for the stretched out appearance the large implants had left her. She used to have AA cup-sized breasts prior to implants and now she is a full A/small B cup size that she’s excited to have.

This 45 year-old patient had her implants placed two decades ago and developed capsular contracture in the recent years. She felt tight in her right breast and it led to more asymmetry of her breasts. She came into the office hoping to get rid of the tightening effect of capsular contracture and restore breast symmetry. She also wanted her natural breast shape back and did not want implants anymore. Mainly she did not want more surgeries in the future. However, she was worried about how small she would be after the implant removal. She also knew implants stretch out the overlying tissue and leave a hollow appearance once the implants are removed. To help boost her post-removal volume and shape, we discussed transferring fat to the breasts. She underwent implant removal (300cc saline implants were removed) and capsulectomy to remove the thick hardened capsule. She had 170cc of fat grafted into the right breast and 100cc into the left breast to make up for the difference in the breast size. She was excited to have symmetrical breasts again and love her natural breasts that feel lighter and beautiful.

This 48-year-old patient had saline breast implants placed over a decade ago and recently experienced a spontaneous rupture of the right implant, further worsening her breast asymmetry. She came to our office wanting to remove her implants and restore a natural, symmetrical breast shape without replacement. She also noted that her breasts had been asymmetrical even before the rupture. To better assess the asymmetry and plan her surgery, we first deflated the left saline implant in clinic. Few weeks later, she underwent implant removal and capsulectomy, followed by fat grafting to both breasts to restore volume and shape. Additionally, a gentle breast lift procedure was performed on the right breast to achieve better symmetry with the left. She is thrilled with her results—her breasts now feel lighter, more natural, and beautifully balanced.
